Join us in person at the Blue Cross Blue Shield of Arizona on October 10th or November 7th for a Comprehensive Health Assessment Event.

You will meet one-on-one with a board-certified nurse practitioner from Adobe Population Health who will:

  • complete your comprehensive health assessment
  • review your medications and answer health-related questions
  • suggest important screenings and tests

Earn $50 in rewards on your reloadable Blue Cross Blue Shield of Arizona (AZ Blue) OTC/Rewards Benefits Card Mastercard® for completing your assessment at this event.

Each member will need to sign up individually to reserve your assessment time slot.
