Join us in person at the Blue Cross Blue Shield of Arizona on September 11 or October 16 for a Comprehensive Health Assessment Event.
You will meet one-on-one with a board-certified nurse practitioner from Homebase Medical who will:

- complete your comprehensive health assessment
- review your medications and answer health-related questions
- suggest important screenings and tests
Earn $40 in rewards on your reloadable Blue Cross Blue Shield of Arizona (AZ Blue) OTC/Rewards Benefits Card Mastercard® for completing your assessment at this event.
Each member will need to sign up individually to reserve your assessment time slot.
